Modification Application Instructions – Quads

Application package includes:

·        Unit Outline Plan – Quads

 

General Note

·        There is often much back and forth communication when going through the Modification Application process.  Once all the pertinent forms and information that are required is agreed upon, a final submission including said forms and information is required in one application packet.  Packets can be emailed to jmion@weaverhomes.com , faxed to (724) 625-7808, mailed to PO Box 449, Mars, PA 16046 or dropped off at our office.

 

Modification Application Form

–       Provide owner name(s), address, email and phone number.

–       Write a detailed description of the modification in the Modification Details box.  Include details such as color, style and dimensions and any other information that may not be included in any drawings, photos or contractors’ quotes.

Unit Outline Plan – Quads

–       Find your unit type and sketch the proposed location of the modification.  Include dimensions, particularly when extending concrete walk or patios.  The orientation on the plan may not match your unit.

Provided By Homeowner

–       Any modifications beyond the sidewalk and/or patio must be shown on a plot plan that shows grading lines and any possible easements that may exist.

–       Photos, brochures, website links that provide building and material specification are required.  They also give us a visual representation of the proposed modification.

–       Include quotes or bids from contractors.  Prices may be redacted if you choose.

–       Insurance certificates for the contractor must be provided before a signed modification can be approved.  If you choose a contractor currently working for Weaver Homes, the insurance certification is not needed and we have it on file.
